A 44-year-old male with longstanding ulcerative pancolitis was initially managed with mesalamine and subsequently treated with infliximab and 6-mercaptopurine. Over the years, infliximab trough levels and thiopurine metabolites were consistently subtherapeutic or undetectable, without evidence of anti-infliximab antibodies. His medical therapy was not changed per his preference. …
