Chirurgische Strategien bei TAVI-Komplikationen

Zusammenfassung

Dieser Beitrag gibt einen Überblick über verschiedene Komplikationen der TAVI-Therapie und deren chirurgische Behandlungsmöglichkeiten. Dabei wird unterschieden zwischen akuten, intraprozedualen und perakuten oder Spätkomplikationen. Konversion zur Sternotomie und Perikardtamponade sind den Frühkomplikationen zuzuordnen und erfordern je nach Ursache und Schwere der Verletzung interventionelle oder primär chirurgische Behandlungsansätze. Bei schweren Akutkomplikationen wie Ventrikelperforation, Anulusruptur und Aortendissektion werden jeweils adäquate und in der Invasivität gestufte chirurgische Konzepte, beginnend mit der dringlichen mechanischen Kreislaufunterstützung bis hin zur komplexen Aortenwurzel- und Aortenchirurgie, vorgestellt und diskutiert. Daneben werden interventionelle Alternativen auch zu weiteren Akutkomplikationen wie der Verlegung von Koronarabgängen, para- und intravalvulärer Aorteninsuffizienz und Fehllage von TAVI-Prothesen berücksichtigt. Von besonderer Bedeutung erscheint hier die enge interdisziplinäre Abstimmung der optimalen interventionellen und/oder chirurgischen Behandlungsstrategie. Im zweiten Teil wird die Prothesenendokarditis als häufigste Spätkomplikation der TAVI-Therapie vorgestellt. Dabei wird auf prothesenspezifische chirurgische Risiken, wie das gehäufte Auftreten ausgedehnter anulärer Abszesse, eingegangen. Die Zugangsplanung zur operativen Behandlung postinterventioneller myokardialer Pseudoaneurysmen erfordert u. a. eine umfangreiche präoperative Bildgebung. Abschließend wird das operative bzw. interventionelle Vorgehen bei subakuter Anulusruptur oder der sehr seltenen Spätmigration oder Kompression von TAVI-Prothesen vorgestellt.
