Though some of his writing tended toward the cryptic, Olli Miettinen was a deep thinker who tried to clarify the fundamentals of epidemiology. In this classic paper [1] he returned to the basic ideas of synergy and antagonism of two exposures, in order to clarify both the “ontology and epistemology” of those phenomena. What should we mean by those words in relation to a binary outcome, and how might we investigate those phenomena in populations? …
