Divertikel des Ösophagus

Ösophagusdivertikel sind seltene Strukturanomalien des Ösophagus, bei denen es zu einer Aussackung mit Kontakt zum Ösophaguslumen kommt. Man unterscheidet echte Divertikel von Pseudodivertikeln. Hinsichtlich ihrer Lokalisation werden die Divertikel in Zenker-Divertikel, epiphrenische Divertikel und Divertikel des mittleren Ösophagus eingeteilt. Wegweisend für die Diagnose des Ösophagusdivertikels ist die Ösophagogastroduodenoskopie. Eine Therapie ist grundsätzlich nur bei symptomatischen Divertikeln erforderlich. Beim symptomatischen Zenker-Divertikel war die kollare Divertikulektomie mit Myotomie nach Aubin lange das chirurgische Standardverfahren, inzwischen wird zunehmend die endoskopische Septotomie durchgeführt. Die Standardtherapie für hochsymptomatische Divertikel des mittleren oder distalen Ösophagus ist die chirurgische Divertikulektomie.
