Malnutrition (Unterernährung) bei Kindern und Jugendlichen

Der über einen längeren Zeitraum bestehende Mangel an Energie und/oder Protein führt beim Menschen, insbesondere bei Kindern, zur Unterernährung, die immer mit Gedeihstörung einhergeht. Hiervon betroffen kann das Gewicht und die Länge sein mit der Entwicklung von Untergewicht und Unterlänge gemessen am Perzentilkurvenverlauf des Kindes. Schwere Unterernährung wird heute im internationalen Sprachgebrauch als Malnutrition bezeichnet. Da es sich bei den hungernden Kindern in der Dritten Welt um eine Kombination aus Proteinmangel und Energiemangel handelt, wird sie auch als Protein-Energie-Malnutrition (PEM) bezeichnet. In der deutschen Sprache verwenden wir dafür die Begriffe Dystrophie oder Atrophie. Bei Kindern in der Dritten Welt mit schwerer Magerkeit („Wasting“) und Längenentwicklungsstörung („Stunting“) sprechen wir von Marasmus. Sind schwere Ödeme vorhanden und Hautstörungen zusammen mit Gedeihstörung, wird dies als Kwashiorkor bezeichnet. Eine Sonderform des Kwashiorkor ist der marantische Kwashiorkor, der sich vom Marasmus durch das gleichzeitige Vorhandensein von Ödemen unterscheidet.
