Veränderungen der Muskarinrezeptoren der alternden Harnblase

Zusammenfassung

Die Prävalenz des Syndroms der überaktiven Blase nimmt mit dem Alter zu. Leider sind die Behandlungserfolge der medikamentösen Therapie oft unbefriedigend. Da Antimuskarinika zurzeit die einzige erwiesenermaßen wirksame Medikation darstellen, liegt die Beteiligung der Muskarinrezeptoren an der Pathogenese dieses Symptomkomplexes auf der Hand. Muskarinrezeptoren sind sowohl in die normale, als auch in die überaktive Detrusorkontraktion involviert und altersbedingte Veränderungen in der Rezeptorfunktion wurden im menschlichen Detrusor sowie im Tiermodell nachgewiesen.
Bei Tieren und auch beim Menschen wurden 2 Komponenten der Miktionskontraktion demonstriert: Der absolut dominierende Anteil wird durch Muskarinrezeptoren vermittelt, der andere durch ATP. Die muskarinerge Komponente nimmt mit zunehmendem Alter ab und der ATP-vermittelte (purinerge) Anteil gewinnt an Bedeutung. Die klinische Bedeutung dieses Phänomens ist unklar, da keine wesentlichen alterbedingten Änderungen der Wirksamkeit antimuskarinischer Medikamente nachgewiesen werden konnten.

Die Hybrid-DRG-Verordnung soll dazu führen, dass mehr chirurgische Eingriffe ambulant durchgeführt werden, wie es in anderen Ländern schon länger üblich ist. Die gleiche Vergütung im ambulanten und stationären Sektor hatten Niedergelassene schon lange gefordert. Aber die Umsetzung bereitet ihnen doch Kopfzerbrechen.

Frauen mit Belastungsinkontinenz oder Organprolaps sind nach einer Netz-Operation keineswegs beschwerdefrei. Vielmehr scheint die Krankheitslast weiterhin hoch zu sein, sogar höher als von harninkontinenten Frauen, die sich nicht haben operieren lassen.

Nun gibt es auch Resultate zum Gesamtüberleben: Eine adjuvante Pembrolizumab-Therapie konnte in einer Phase-3-Studie das Leben von Menschen mit Nierenzellkarzinom deutlich verlängern. Die Sterberate war im Vergleich zu Placebo um 38% geringer.
