A 61-year-old male, with ulcerated cecum adenocarcinoma and lymph nodes, bone and hepatic metastases (Stage IV, Grade 3), presented with fever (T 38.1 °C), fatigue, poor clinical condition associated with back pain when standing. Vertebral CT-guided palliative care radiotherapy, was programmed. Blood tests were remarkable for leukocytosis (20,330/µl, N.V 4500–11,000/µl) with neutrophilia (16,700/µl, N.V 2500–7500/µl), C-reactive protein 332 mg/L (N.V. 0.0–0.5 mg/L), procalcitonin 10.50 µg/L (N.V < 0.5 µg/L).
