A dialysis catheter was uneventfully inserted into the left internal jugular vein under routine ultrasound guidance. However, the anteroposterior chest X-ray revealed the Shaldon catheter projecting on the descending thoracic aorta, suggestive of aortal misplacement (Fig. 1). The consecutive CT scan revealed a venous placement in the accessory hemiazygos vein just behind the thoracic aorta (Figs. 2, 3). Under fluoroscopy, the catheter was replaced into the superior vena cava.
