174. Akutes Leberversagen und Lebertransplantation

Zusammenfassung

Das pädiatrische akute Leberversagen (pALV) ist selten und potenziell reversibel. Definiert ist es durch biochemische Zeichen einer Lebererkrankung und eine Vitamin K unabhängige Gerinnungsstörung. Eine hepatische Enzephalopathie ist bei pALV keine Conditio sine qua non. Die Ursache ist äußerst variabel und altersabhängig. Ein pALV ist meist durch unspezifische Symptome eines Leberfunktionsverlustes gekennzeichnet, wobei das erste hinweisende Symptom in der Regel ein Ikterus ist. Die pädiatrische Lebertransplantation (pLTx) ist das Standardverfahren im Endstadium von akuten und chronischen Lebererkrankungen, Lebertumoren sowie hepatisch bedingter Stoffwechselerkrankungen. Dabei werden an großen, spezialisierten pädiatrischen Lebertransplantationszentren bei guter Lebensqualität Langzeitüberlebensraten von 85–90 % erreicht. Dieses wurde möglich durch Einführung moderner Immunsuppressiva und von Operationstechniken, die die LTx von an die Größe der Kinder adaptierten Leberteilen ermöglichen.
