Risikoadjustierte Nachsorge nach endovaskulärer Behandlung abdomineller infrarenaler Aortenaneurysmen?

Zusammenfassung

Die endovaskuläre Therapie hat sich zur primären Behandlungsoption für elektive abdominale Aortenaneurysmen entwickelt. Im Langzeitverlauf muss sich ein Viertel der Patienten nach EVAR jedoch sekundären Interventionen unterziehen, um einen adäquaten Aneurysmaausschluss aufrechtzuerhalten und letztlich negative aneurysmaassoziierte Folgen (eine sekundäre Aneurysmaruptur nach EVAR in 7 % der Fälle) zu verhindern. Um den Behandlungserfolg aufrechtzuerhalten und ggfs. eine zeitnahe sekundäre Intervention anzubieten, wurde in den bisherigen Leitlinien eine intensive Bildgebung empfohlen. Diese strengen Überwachungsverfahren sind kostspielig und können schädliche strahlen- und kontrastmittelassoziierte Nebenwirkungen haben. Verbesserungen bei der Patientenauswahl, der operativen Technik und den Stentprothesen haben zu einem Rückgang der Komplikationen nach EVAR geführt. Daher besteht die Notwendigkeit, die Überwachung nach EVAR für Patienten mit geringerem Komplikationsrisiko zu verringern und individualisierte Folgeprotokolle zu installieren. Anhand der Ergebnisse der aktuellen Literatur werden Strategien, adaptiert an Risikofaktoren und Prädiktoren für sekundäre Komplikationen, für die langfristige postoperative Überwachung nach EVAR diskutiert.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
