In extremity and trunk wall soft tissue sarcoma (STS), the microscopic status of surgical margins is the benchmark for defining the quality of local treatment. Since the late 1970s, the standard approach has consisted of wide margin resection followed by radiation therapy (RT) in case of tumor at significant risk of recurrence, i.e. high malignancy grade, deep location, diameter > 5 cm, and positive microscopic surgical margins not suitable for further improvement without significantly compromising function/cosmesis. The setting of adjuvant RT, whether pre- or postoperatively, has shown to be irrelevant for its effect. …
