Zusammenfassung

Patienten mit Borderline-Störungen leiden unter komplexen Problemen: Meist sind Emotionsregulation, soziale Interaktion, Selbsthass und häufig auch das Trauma-Gedächtnis betroffen. Die psychotherapeutische Behandlung erfordert dementsprechend differenzierte klare modulare Konzepte, die jedoch individualisierte Behandlungsplanung zulassen. Die Wirksamkeit der Dialektisch Behavioralen Therapie (DBT) konnte in einer Vielzahl von Studien nachgewiesen werden, und gilt daher als Therapie der Wahl. Die DBT gliedert sich in 3 Therapiestadien mit unterschiedlichen Schwerpunkten: Im Zentrum des ersten Stadiums steht die Vermittlung von Fertigkeiten zur Kontrolle über lebensgefährliche und krisenerzeugende Verhaltensmuster; in der zweiten Phase werden schwerwiegende Störungen behandelt, die die emotionale Lernfähigkeit beeinträchtigen; die dritte Phase konzentriert sich auf borderlinetypische emotionale Automatismen wie Einsamkeit, Angst vor Verlassenwerden oder Selbsthass.
