A 27-year-old woman presented with single generalized tonic-clonic seizure, diplopia and headache. Neurologic examination showed bilateral facial and oculomotor palsy. She had HIV infection diagnosis 7 years before but was not under treatment with highly active anti-retroviral therapy (HAART). The CT scan was unremarkable. Lumbar puncture was performed and cerebrospinal fluid revealed 13 cells/mm3 (mainly mononucleated), 101 mg/dL protein, 9 mg/dL glycorrhachia and opening pressure 510 mm/H2O. Plasma viremia by RT-PCR of 183.126 HIV-1 RNA copies/ml and 7 CD4+ cells/mm3. CSF cultures to Cryptococcus neoformans and CSF PCR parvovirus B 19 were positive. CSF culture for Mycobacterium tuberculosis and CSF-VDRL for Treponema pallidum were negatives. …
