Auf einen Blick

Verordnungsprofil Größte Gruppe der Antidementiva sind die Acetylcholinesterase-Inhibitoren (Cholinesterasehemmer) gefolgt von dem NMDA-Rezeptorantagonisten Memantin, der aber nur etwa halb so viel verordnet wird. In beiden Gruppen sind nur noch Generika vertreten. Traditionelle Antidementiva (Piracetam, Ginkgoextrakt, Nicergolin) ohne gesicherten Nutzen werden immer noch verschrieben. Nicht mehr vertreten ist Riluzol, das v. a. zur neuroprotektiven Therapie der amyotrophen Lateralsklerose eingesetzt wird.

Bewertung Der symptomatische Nutzen der besprochenen Arzneistoffe ist insgesamt begrenzt. In der S3-Leitlinie der Deutschen Gesellschaft für Psychiatrie, Psychotherapie und Nervenheilkunde und Deutschen Gesellschaft für Neurologie 2016 wird bei leichter bis mittelschwerer Alzheimer-Demenz die Gabe eines Cholinesterasehemmers in der höchsten verträglichen Dosis empfohlen; ebenso wird bei Krankheitsprogredienz eine Beibehaltung der Therapie empfohlen (Off-Label-Gebrauch).
