109. Diabetischer Fuß

Zusammenfassung

Das Krankheitsbild des diabetischen Fußes ist definiert als eine Infektion, Ulzeration und/oder Zerstörung des tiefen Weichteilgewebes mit neuropathologischen Befunden und einer unterschiedlich ausgeprägten peripheren Gefäßerkrankung der unteren Extremitäten (Schaper et al. 2003; International Working Group on the Diabetic Foot 2007). Wundheilungsstörungen der unteren Extremitäten bei einer diabetischen Stoffwechsellage stellen eine besondere Herausforderung dar. Statistisch gesehen wird weltweit alle 30 Sekunden eine dem Diabetes mellitus anzulastende Amputation an einer unteren Extremität durchgeführt. Weiterhin zeigt sich, dass ca. 20–40 % der Gesamtkosten einer Diabeteserkrankung auf die Therapie des diabetischen Fußsyndroms entfallen (Boulton et al. 2005). Studien belegen, dass 50–70 % aller Amputationen der unteren Extremitäten einer diabetischen Stoffwechsellage zuzuschreiben sind. Bei 85 % aller Amputationen, die an Diabetes mellitus erkrankten Patienten durchgeführt wurden, ging ein Fußulkus voraus, das im weiteren Verlauf eine schwere Infektion oder Gangrän ausbildete (International Working Group on the Diabetic Foot 2007; Boulton et al. 2005; Apelqvist und Larsson 2006).

Seit November 2023 gibt es evidenzbasierte Empfehlungen zum perioperativen Management bei gastrointestinalen Tumoren (POMGAT) auf S3-Niveau. Vieles wird schon entsprechend der Empfehlungen durchgeführt. Wo es im Alltag noch hapert, zeigt eine Umfrage in einem Klinikverbund.

Die Hybrid-DRG-Verordnung soll dazu führen, dass mehr chirurgische Eingriffe ambulant durchgeführt werden, wie es in anderen Ländern schon länger üblich ist. Die gleiche Vergütung im ambulanten und stationären Sektor hatten Niedergelassene schon lange gefordert. Aber die Umsetzung bereitet ihnen doch Kopfzerbrechen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
