Endophthalmitis

Zusammenfassung

Eine Endophthalmitis ist eine schwere Infektion des Augapfels mit der Gefahr einer Erblindung oder eines Augapfelverlusts. Man unterscheidet eine exogene (postoperative oder posttraumatische) Endophthalmitis von der endogenen Endophthalmitis. Häufigster Grund einer exogenen Endophthalmitis ist eine vorausgegangene Kataraktoperation oder eine intravitreale Medikamenteninjektion. Typische Symptome sind Visusverlust und Schmerzen. Hypopyon und Glaskörperinfiltration mit Verlust des Fundusrots sind die wichtigsten klinischen Befunde für eine Endophthalmitis. Die Diagnose ist primär klinisch, die mikrobiologische Diagnostik aus Vorderkammer und/oder Glaskörper unterstützt die Diagnose und hilft bei einer zielgerichteten Therapie. Basis der Therapie ist die Gabe von intraokulären, in der Regel intravitrealen Antibiotika, oft in Kombination mit einer Vorderkammerspülung und/oder Vitrektomie. Systemische Antibiotika können unterstützend gegeben werden. Der entscheidende Faktor für die Prognose ist der auslösende Keim. Infektionen mit koagulasenegativen Staphylokokken oder Candida spp. haben eine gute Prognose. Infektionen mit Streptokokken, Bacillus spp. oder Aspergillus spp. sind ungünstig.
