The incidence of accessory spleen is approximately 10% in autopsy findings, with the most frequent location being at the tail of the pancreas [

2]. Epidermoid cysts arising from ectopic spleen tissue in the tail of the pancreas are a benign and extremely rare pathology. At present, less than 50 cases were being reported in the English literature.
