Hautadnex- vs. Speicheldrüsentumoren

Zusammenfassung

Trotz gravierender Unterschiede in der zugrunde liegenden mikroskopischen Anatomie bestehen innerhalb des weiten Spektrums von Hautadnex- und Speicheldrüsentumoren bemerkenswerte Ähnlichkeiten. In der vorliegenden Arbeit werden Typen von Speicheldrüsen- und Hautadnextumoren mit histologischen, terminologischen und pathogenetischen Analogien identifiziert und verglichen. Histologische Analogien von Speicheldrüsentumoren beziehen sich auf ekkrine, apokrine und selten sebozytäre (nicht trichofollikuläre) Typen von Hautadnextumoren. Die auffälligste Analogie besteht zwischen dem pleomorphen Adenom der Speicheldrüsen und dem kutanen Mischtumor. Eine Mehrschrittkanzerogenese, beginnend mit einem intraduktalen Karzinom, welche beim Karzinom ex pleomorphes Adenom identifiziert wurde, ist identisch manifest beim kutanen Karzinom ex Spiradenom. Weitere histologische und/oder terminologische Analogien werden aufgezeigt für muzinöse/mukoepidermoide Karzinome, Lymphadenome/lymphoepitheliale Karzinome, sebazeöse Adenome bzw. Karzinome, adenoidzystische Karzinome sowie für Basalzelladenome der Speicheldrüsen vs. kutane Spiradenome. Manifeste diagnostische Probleme bzgl. histologisch ähnlicher Speicheldrüsen- und Hautadnextumoren sind selten und topographisch beschränkt auf die parotideale und orale Region.
