Zusammenfassung

Heutzutage überleben die meisten Patienten mit angeborenen Herzfehlern (AHF) bis ins Erwachsenenalter. Leider wird die Nachsorge von Erwachsenen mit angeborenen Herzfehlern (EMAH) trotz relevanter Residuen und Folgeerkrankungen überwiegend von nicht spezialisierten und im EMAH-Bereich wenig erfahrenen Ärzten oder Kliniken durchgeführt. Zu den wichtigsten Problemen im Langzeitverlauf gehören vitienspezifische Resieduen, Herzinsuffizienz, Herzrhythmusstörungen, Herzklappenerkrankungen, pulmonale Gefäßerkrankungen, infektiöse Endokarditis, Aortopathie sowie kardiale und nichtkardiale Komorbiditäten. Viele kardiale Probleme bei EMAH manifestieren sich anders als bei erworbenen Herzkrankheiten, und Therapieschemata aus der allgemeinen Kardiologie können nicht unmittelbar auf AHF übertragen werden. Es sei darauf hingewiesen, dass selbst einfache, postoperative Herzfehler, die bis vor Kurzem noch als harmlos galten, mit zunehmendem Alter ernsthafte, vitienbedingte Komplikationen entwickeln können, eine Tatsache, die bisher kaum erwartet wurde. Die Behandlung der ACHD weist daher viele Besonderheiten auf und erfordert besonderes Fachwissen. Das vorliegende Positionspapier der Deutschen Gesellschaft für Kardiologie gibt einen hochaktuellen Überblick und liefert aktuelle Daten zur Epidemiologie, Herzinsuffizienz und Herzrhythmusstörungen bei EMAH.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Somit können jetzt alle Patient:innen mit HF von SGLT-2i als zentralem Bestandteil der Therapie profitieren.

Dapagliflozin als neue Therapieoption zur Behandlung einer Herzinsuffizienz unabhängig von der Ejektionsfraktion

Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.

ACS-Erstlinientherapie: Konsensbeschluss rät zur DAPT mit Ticagrelor

Ein Konsortium führender Fachgesellschaften erarbeitete jüngst auf Basis umfangreicher Metaanalysen einen Konsens für die Therapie koronarer Herzkrankheiten. Was dabei auffiel: Die duale Plättchenhemmung (DAPT) mit Ticagrelor ist die bevorzugte Therapieoption für das akute Koronarsyndrom (ACS).
