Hintergrund

In Dänemark werden Patienten durchschnittlich 2 Tage nach Knie- und Hüfttotalendoprothese entlassen. Durch ein konsequentes Untersuchen traditioneller Behandlungsmethoden und ein zielgerichtetes Forschen, ist es in den letzten 20 Jahren gelungen, den Behandlungsverlauf so zu optimieren, dass in der Folge die Krankenhausverweildauer stark reduziert werden konnte.

Grundlagen des Fast-Track-Modells

Die wichtigsten Elemente dieses Fast-Track-Modells werden beschrieben – insbesondere die Motivierung der Patienten und die Übertragung von Mitverantwortung durch intensive Patientenaufklärung, optimierte Operationstechniken sowie moderne multimodale Schmerztherapie mit Frühmobilisierung. Das relativ kleine und homogene Gesundheitswesen Dänemarks besitzt zum einen gute Forschungsbedingungen mit der Möglichkeit einer raschen Umsetzung der neuesten Forschungsergebnisse und zum anderen ein auf Fallpauschalen beruhendes Vergütungssystem ohne Grenzverweildauer – zwei unterschiedliche Faktoren, die beide auf ihre Weise die Entwicklung des Fast-Track-Modells begünstigt haben.
