Indikationen und Operationstechniken für kombinierte mediale und laterale Eingriffe bei schwerer extrinsischer Ellenbogensteife

Zusammenfassung

Auch ausgeprägte extrinsische Ellenbogengelenksteifen sind mit einer offenen Arthrolyse effektiv und mit zufriedenstellenden Ergebnissen zu behandeln. Eine sorgfältige präoperative Planung zur Beurteilung einer möglichen Gelenkbeteiligung und Entscheidung hinsichtlich des zu wählenden Zugangswegs ist von entscheidender Bedeutung. In den meisten Fällen ist bei ausgedehnter Beteiligung der periartikulären Weichteile ein kombinierter lateraler und medialer Zugang zu bevorzugen, wobei sich der Umfang des Eingriffs schrittweise an der intraoperativ erzielten Zunahme des Bewegungsausmaßes orientiert. Der N. ulnaris sollte in aller Regel identifiziert und in den meisten Fällen nach ventral verlagert werden. Intraoperativ muss eine substanzielle Verbesserung des Bewegungsumfangs erzielt werden, da postoperativ nahezu immer mit einem erneuten Verlust der Beweglichkeit zu rechnen ist. Eine unmittelbar postoperativ eingeleitete statische Schienenbehandlung ist einer aktiven Physiotherapie vorzuziehen. Durch die operative Arthrolyse kann bei extrinsischer Ellenbogengelenksteife und konsequentem Vorgehen mit einem absoluten Zugewinn des Bewegungsumfangs zwischen 30 und 60° gerechnet werden.
