Primary small bowel adenocarcinoma is an uncommon malignancy and most of us recall that, of gastrointestinal cancers, small bowel adenocarcinoma constitutes a single-digit percentage of less than 5%.

2 Similarly recalled is the dismal prognosis for patients with this diagnosis, and among the reasons cited are the nonspecific symptoms reported prior to frank obstruction or hemorrhage, the difficulty in obtaining tissue for diagnosis from lesions distal to the duodenum, and the under-staging of the disease by conventional modalities.
