Zusammenfassung

Die kardiale Computertomographie mit Kalziumscoring zur Quantifizierung des Koronarkalks und kontrastmittelgestützter CT-Angiographie (cCTA) zur nichtinvasiven morphologischen und funktionellen Beurteilung von Koronarstenosen ist ein etabliertes Verfahren in der Diagnostik der koronaren Herzerkrankung (KHK) und der zielgerichteten interventionellen Behandlung. Die kardiale Computertomographie eignet sich bei Patienten mit niedriger bis intermediärer Vortestwahrscheinlichkeit zur Risikostratifizierung (Kalziumscoring) und zum Ausschluss einer KHK mittels cCTA. Die rein anatomische Darstellung von Koronarstenosen kann zukünftig durch funktionelle Analysen aufgrund neuer technischer Entwicklungen ergänzt werden und erlaubt somit neben einer morphologischen Beurteilung eine Aussage über die hämodynamische Signifikanz von Koronarstenosen. Durch die vielversprechende kombinierte anatomische und funktionelle Diagnostik könnte der kardialen Computertomographie in Zukunft eine wesentliche Gatekeeper-Funktion hinsichtlich der Indikationsstellung für Koronarinterventionen zukommen.
