Anästhesiologisch relevante Aspekte der ESC-Leitlinien für das Management von kardiovaskulären Erkrankungen während der Schwangerschaft (Update 2018)

Zusammenfassung

Die vorliegende ESC(European Society of Cardiology)-Leitlinie zur Betreuung von Schwangeren mit kardiovaskulären Erkrankungen soll den behandelnden Ärzten einen Handlungsleitfaden bieten. In den westlichen Ländern zählen Herzerkrankungen zu den häufigsten Todesursachen von Frauen während einer Schwangerschaft. Neben dem Themenkomplex kongenitaler Herzfehler und pulmonalarterieller Hypertonus wird u. a. auf Aorten- und Klappenerkrankungen sowie auf Arrhythmien und auf den arteriellen Hypertonus während der Schwangerschaft eingegangen. Unterschiede zu der Leitlinie von 2011 sind beispielsweise die modifizierte Klassifikation des mütterlichen Risikos gemäß der mWHO(modified World Health Organization)-Klassifikation sowie die differenzierte Empfehlung zur Antikoagulation bei Schwangeren mit niedrigen und hohen Dosen an Vitamin-K-Antagonisten. Der Fokus dieser Zusammenfassung liegt auf anästhesiologisch relevanten Aspekten zur situationsadäquaten Vorbereitung des zuständigen geburtshilflichen Anästhesisten.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Somit können jetzt alle Patient:innen mit HF von SGLT-2i als zentralem Bestandteil der Therapie profitieren.

Dapagliflozin als neue Therapieoption zur Behandlung einer Herzinsuffizienz unabhängig von der Ejektionsfraktion

Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.
