Intestinale Perfusionsstörungen beim Intensivpatienten

Zusammenfassung

Intestinale Perfusionsstörungen stellen wegen der geringen Hypoxietoleranz des Darmes eine schwere Erkrankung hoher Mortalität dar. Sie begegnen dem perioperativ tätigen Intensivmediziner im Zusammenhang mit anderen schweren Grunderkrankungen, als Folge chirurgischer Eingriffe, unter Medikamentengabe oder invasiver Therapie mit organersetzenden Verfahren. Von zunehmender intensivmedizinischer Bedeutung ist die nonokklusive Form der mesenterialarteriellen Ischämie (NOMI). Aufgrund der reduzierten Symptomatik bei Intensivpatienten erfordert sie eine erhöhte Aufmerksamkeit. Entscheidend für die Prognose ist die frühzeitige Diagnose. Der Übersichtsartikel stellt neben der Pathophysiologie die Diagnostik und das Spektrum therapeutischer sowie präventiver Maßnahmen vor.
