Neoadjuvante Therapie von Plattenepithelkarzinomen des Ösophagus

Was ist „evidence based“?

Zusammenfassung

Die neoadjuvante Therapie lokal fortgeschrittener Plattenepithelkarzinome des Ösophagus ist weit verbreitet. Nach Kriterien der „evidence based medicine“ gibt es jedoch keinen klaren Nachweis für ihren Stellenwert. Die Effekte einer neoadjuvanten alleinigen Strahlentherapie sind allenfalls gering einzuschätzen. Neoadjuvante Chemotherapie alleine führt ebenfalls nur zu einer geringen Prognoseverbesserung. Für die kombinierte Radiochemotherapie zeigen zwar alle Einzelstudien im Hinblick auf den Endpunkt Überleben keinen statistisch signifikanten Gewinn, die Ergebnisse von drei Metaanalysen deuten aber insgesamt einen positiven Effekt an. Die älteren Protokolle waren toxisch und erhöhten das perioperative Morbiditäts- und Sterblichkeitsrisiko. Moderne Radiochemotherapieprotokolle hingegen haben sich an erfahrenen Einrichtungen als gut durchführbar erwiesen und sind bei lokal fortgeschrittenen Tumoren auf alle Fälle indiziert. Die Patienten profitieren davon, wenn die initiale Bahnung der Therapie ebenso wie die Operation an Zentren erfolgen. Die Behandlung sollte wann immer möglich im Rahmen klinischer Studien erfolgen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
