Zusammenfassung

Die Sentinel-Node-Biopsie (SNB, Wächterlymphknotenbiopsie) wird zunehmend als minimal-invasive Stagingmethode bei Patienten mit einem Melanom der Haut eingesetzt. Über Jahrzehnte wurde an vielen Zentren bei Patienten ohne klinisch nachgewiesene Lymphknotenmetastasen, bei denen jedoch von einem erhöhten Metastasierungsrisiko ausgegangen wurde, eine elektive (prophylaktische) Lymphknotendissektion (ELKD) praktiziert. Dagegen werden heute Patienten nur bei einem nachgewiesenen Befall des Sentinel Node (10–29%) einer (selektiven) Lymphknotendissektion (SLKD) zugeführt. Der metastatische Befall des Wächterlymphknotens ist einer der wichtigsten Prognosefaktoren. Der Stellenwert der SNB in der Behandlung von Melanompatienten wird auch 10 Jahre nach Einführung der Methode sehr kontrovers diskutiert. Unklar sind die Bedeutung von hochsensitiven molekularbiologischen oder zytologischen Techniken zum Metastasennachweis wie auch der therapeutische Nutzen der SNB per se sowie der anschließenden operativen und/oder medikamentösen adjuvanten Therapien.
