Messung des intraabdominellen Drucks

Zusammenfassung

Das Vorliegen einer intraabdominellen Hypertension oder gar eines abdominellen Kompartmentsyndroms verschlechtert die Prognose chirurgischer und internistischer Intensivpatienten. Die intraabdominelle Hypertension ist ab einem intraabdominellen Druck von ≥12 mmHg definiert, ein abdominelles Kompartmentsyndrom liegt bei einer anhaltenden Druckerhöhung >20 mmHg mit neu aufgetretener Organdysfunktion/-versagen vor. Ein erhöhter intraabdomineller Druck kann zu einer Einschränkung des kardiorespiratorischen Systems als auch der Nierenfunktion und der abdominellen Perfusion führen und hat Auswirkungen auf entfernt liegende Organsysteme. Der Goldstandard zur Messung des intraabdominellen Drucks ist die Bestimmung des Blasendrucks. Die Messung sollte in flacher Rückenlage nach Instillation von 25 ml steriler Kochsalzlösung erfolgen. Zur Therapie bei intraabdomineller Hypertension und beim abdominellen Kompartmentsyndrom stehen zahlreiche konservative Methoden, deren Effektivität allerdings erst noch in Studien gezeigt werden muss, zur Verfügung. Bei therapierefraktärem abdominellem Kompartmentsyndrom sollte schließlich die entlastende Laparotomie mit temporärem Bauchdeckenverschluss angestrebt werden.
