Multimorbidität bei älteren Rheumapatienten Teil 1

Zusammenfassung

Um eine sachgerechte Versorgung des älteren Rheumapatienten zu gewährleisten, sind nicht nur häufig eine umfassende Diagnostik und eine Ausschlussdiagnostik von z. B. koronarer Herzerkrankung, Osteoporose, Niereninsuffizienz, Diabetes mellitus Typ 2 wie auch Schilddrüsendysfunktionen notwendig, sondern es kann auch aufgrund der komplexen Behandlungssituation die Einbindung mehrerer Fachrichtungen sowie eine akutstationäre Versorgung erforderlich sein.

Die Komplexität von Multimorbidität bei älteren Rheumapatienten wächst nicht nur mit dem Alter, sondern stellt zudem eine erhebliche Herausforderung aufgrund der zumeist vorliegenden Fähigkeitsstörungen wie auch aufgrund der zurückliegenden und der aktuell durchgeführten immunsuppressiven Therapie dar. Der hier notwendige Behandlungsrahmen wird aus Sicht des Rheumatologen und des Geriaters aufgezeigt.

Geriatrietypische Symptome wie Sarkopenie, Malnutrition, Immobilität und „frailty“ (Altersgebrechlichkeit) können bei gleichzeitig vorliegender Multimorbidität noch verstärkt werden.
