Natural orifice transluminal endoscopic surgery (NOTES) and single-port access surgery are significant new developments, surely not the last, in the continuing evolution of minimally invasive therapies. The potential these advances may hold for improved clinical outcomes is just beginning to be realized. Minimally invasive surgeons have already begun to use single-incision access to perform a range of clinical procedures, with little to no formal guidance. Despite design shortcomings, currently available ports, instruments, optics, and supplemental technologies such as robotics have been used successfully to obtain initial experiences with these novel approaches. …
