Inguinal hernia represents a frequent disease with plenty of clinical manifestation. In the majority of cases, the hernia content consists in a segment of small bowel or lipomas, less commonly large bowel or bladder but unusual conditions has also been reported. Incarcerated sigmoid colon is a rare condition as well. More uncommon is the occurrence of perforation of a diverticulum of the sigmoid colon incarcerated into an inguino-scrotal hernia and only few cases have been reported so far. Kouraklis, in the 2004 quoted the first knowing case in the English literature [
1]. Herein, we present a case of localized peritonitis due to diverticular perforation find in an inguinal hernia sac with relevant literature to illustrate uncommon clinical case and underlining preoperative diagnosis and surgical aspects. …
