Ektopien der Hypophyse

Zusammenfassung

Die Adenohypophyse hat einen weit überwiegenden intrasellären Hauptteil und weitere recht kleine Anteile am Hypophysenstiel und am Rachendach. In deren naher Umgebung, aber auch im Sinus sphenoidalis und im Sinus cavernosus können sich ektope Hypophysenadenome ausbilden, die sich strukturell und funktionell nicht von den üblichen intrasellären Adenomen unterscheiden lassen. Die Neurohypophyse kann Dystopien besonders im Rahmen von komplexen Missbildungen zeigen, aber auch beim sog. Pituitary-Stalk-Interruption-Syndrom echte Ektopien an der Eminentia mediana aufweisen. Ektopes Gewebe in der Hypophyse ist extrem selten: Es wurden Ganglienzellnester in der Neurohypophyse und Nebennierenrindengewebe in der Adenohypophyse nachgewiesen. Umschriebene Plattenepithelherde in der Adenohypophyse sind häufig und stellen Metaplasien von Parenchymzellen oder Reste des Ductus craniopharyngeus dar. Ektopien von Speicheldrüsengewebe sind in sehr kleinen Herden in der Intermediärzone nicht selten.

Die Therapie von Echinokokkosen sollte immer in spezialisierten Zentren erfolgen. Eine symptomlose Echinokokkose kann – egal ob von Hunde- oder Fuchsbandwurm ausgelöst – konservativ erfolgen. Wenn eine Op. nötig ist, kann es sinnvoll sein, vorher Zysten zu leeren und zu desinfizieren.
