Neue Entwicklungen in Diagnostik und Therapie

Zusammenfassung

Im vorliegenden Artikel werden neueste Entwicklungen im Therapiealgorithmus des Nierenzellkarzinoms (NZK) diskutiert. Hierbei wird auf das Problem der präoperativen Bildgebung zur Beurteilung der Dignität der Nierenraumforderung, ebenso wie der Nachweis von Metastasen und deren Unterscheidung zu Zweitmalignomen eingegangen. Zur prognostischen Einteilung des NZK wird die geänderte Version des TNM-Systems beschrieben. In Bezug auf die nierenerhaltenden Techniken erfolgt der kritische Vergleich der Laparoskopie mit der Radiofrequenz- und der Kryoablation. Ebenso werden neueste Ergebnisse des roboterassistierten (DaVinci) Nierenerhalts vorgestellt. Spezielle Fragen wie die der zytoreduktiven Operation im primär metastasierten Stadium und die noch ungeklärte Frage nach einer adjuvanten Immuntherapie sind Bestandteil der Diskussion. In der medikamentösen Therapie werden neue zugelassene und sich in Studien befindliche Medikamente erläutert und die Möglichkeiten einer Sequenztherapie diskutiert.

Adjuvante Immuntherapie verlängert Leben bei RCC

Nun gibt es auch Resultate zum Gesamtüberleben: Eine adjuvante Pembrolizumab-Therapie konnte in einer Phase-3-Studie das Leben von Menschen mit Nierenzellkarzinom deutlich verlängern. Die Sterberate war im Vergleich zu Placebo um 38% geringer.

Erst PSA-Test, dann Kallikrein-Score, schließlich MRT und Biopsie – ein vierstufiges Screening-Schema kann die Zahl der unnötigen Prostatabiopsien erheblich reduzieren: Die Hälfte der Männer, die in einer finnischen Studie eine Biopsie benötigten, hatte einen hochgradigen Tumor.

Beim chronischen Harnwegsinfekt bei Frauen wird bisher meist eine Antibiotikaprophylaxe eingesetzt. Angesichts der zunehmenden Antibiotikaresistenz erweist sich das Antiseptikum Methenamin-Hippurat als vielversprechende Alternative, so die Auswertung einer randomisierten kontrollierten Studie.
