Schmerzmedikation in der Schwangerschaft

Zusammenfassung

Bei einer analgetischen Therapie in der Schwangerschaft sind einige Einschränkungen zu berücksichtigen. Paracetamol ist das Mittel der Wahl in jeder Phase der Schwangerschaft bei leichten oder mittleren Schmerzen. Ibuprofen ist das nichtsteroidale Analgetikum der Wahl, wobei diese Substanzen nach der 28. Schwangerschaftswoche wegen der Gefahr eines vorzeitigen Ductusverschlusses und fetaler Nierenfunktionsstörungen kontraindiziert sind. Bei starken Schmerzen können auch Opioide eingesetzt werden, allerdings muss bei peripartaler Anwendung mit Atemdepression und Anpassungsstörungen beim Neugeborenen gerechnet werden, bei einer Dauertherapie bis zum Ende der Schwangerschaft mit Entzugserscheinungen. Migräne kann mit Sumatriptan behandelt werden. Antiepileptika sollten bei Schwangeren nicht angewendet werden, da ein teratogenes Risiko meist nicht auszuschließen ist. Gut untersuchte Antidepressiva wie Amitriptylin können dagegen bei entsprechender Schmerzindikation verwendet werden.
