A 37-year-old obese woman scheduled for placement of a BioEnterics Intragastric Balloon (BIB) underwent sedation with IV propofol 1 mg/kg via an IV catheter. At the end of the infusion, she developed twitching and rhythmic movements of the limbs, with increased muscle tone of the trunk. The seizure-like phenomena resulted in apnoea and rapid desaturation. After undergoing rapid sequence induction with propofol 2 mg/kg and succinylcholine, her airway was secured and oxygenation was re-established. BIB placement was performed successfully under desflurane anaesthesia. Her recovery was routine, and she experienced no further seizures.Author CommentSeizure-like phenomena “represents an unpredictable and insidious adverse event observed mainly during induction of or emergence from sedation or anesthesia with propofol doses ranging between 0.5 and 5.2 mg/kg.” …
