The choice of an optimal surgical treatment for a benign disease should be based on both safety and efficacy or, in other words, the ability to cure the disease with negligible morbidity. Recurrent disease can occur in up to 20% of patients with benign multinodular goiter after subtotal thyroidectomy and reoperation for thyroid bed recurrence is associated with substantial operative risk. Total thyroidectomy is gaining popularity over subtotal thyroidectomy for benign multinodular goiter in many endocrine surgery centers because of the ease of postoperative hormone replacement, the low morbidity rate achieved by experienced surgeons, and, most importantly, the added benefit of almost no risk of recurrence in the longer term.
