Chirurgische Therapie des Karzinoms des unteren Rektumdrittels

Zusammenfassung

Die Therapie des Karzinoms des unteren Rektumdrittels steht im Spannungsfeld zwischen der notwendigen onkologischen Radikalität und dem Erhalt der Kontinenz. Voraussetzung für eine optimale Therapie ist eine subtile präoperative Diagnostik, in dessen Mittelpunkt die proktologische Untersuchung durch den erfahrenen Chirurgen und die Magnetresonanztomographie stehen. Neben der tiefen anterioren Resektion mit koloanaler Rekonstruktion kommen die partielle oder die komplette intersphinktäre Resektion als kontinenzerhaltende Verfahren zum Einsatz. Für die abdominoperineale Exstirpation (APE) stehen die intersphinktäre, die Standard- und die extralevatorische APE zur Verfügung. Neue Operationsverfahren wie die transanale totale mesorektale Exzision (TaTME) und die roboterassistierte TME könnten zu einer Verbesserung der funktionellen und onkologischen Ergebnisse gerade bei Karzinomen des unteren Rektumdrittels beitragen, sind zurzeit aber noch in Erprobung.
