A 73-year-old man developed disseminated, increasingly pruritic and painful skin alterations upon treatment with oral sorafenib 200mg twice daily for metastatic hepatocellular carcinoma induced by hepatitis C virus infection. Examination revealed distinct, slightly atrophic, desquamative, confluent annular macules covering his abdominal skin fold, lower trunk and left thigh. Biopsy of the thigh lesion demonstrated discrete acanthosis, extravasation of erythrocytes, a perivascular lymphocytic infiltrate in the dermis, and hyperparakeratosis with focal accumulation of neutrophilic granulocytes in the epidermis. The skin changes appeared simultaneously with sorafenib treatment and persisted unchanged for a treatment period of 6 months.Author Comment“Several features of this case suggest a direct relationship of the skin changes with sorafenib.” …
