Zusammenfassung

In den letzten 10 Jahren hat sich die Aortenklappenchirurgie in Deutschland verändert. Während 2006 erstmals 0,7 % der Patienten mit der Indikation zum Aortenklappenersatz (AKE) eine „transcatheter aortic valve implantation“ (TAVI) erhielten, beträgt gemäß der DGTHG-Jahresstatistik der Anteil der TAVI im Verhältnis zum konventionellen AKE 2014 46,3 %. Die Zahl der konventionell durchgeführten Aortenklappenoperationen ist über die Jahre sogar leicht gestiegen, von 11.011 Patienten 2005 auf 11.764 im Jahr 2014. Eine deutliche Verschiebung beim konventionellen AKE gab es zwischen 2005 und 2014: Der Anteil der implantierten mechanischen im Vergleich zu biologischen Aortenklappenprothesen sank von 45,6 % auf 13,1 %. Der Anteil der Patienten im Alter zwischen 20 und 70 Jahren ist dagegen von 54,8 % auf aktuell 45,7 % gesunken. Doch, was sind die Auswahlkriterien für eine biologische oder eine mechanische Aortenklappenprothese? Der vorliegende Beitrag unterstützt bei der Wahl der „idealen“ Aortenklappenprothese.
