20. Tissue Engineering der Haut

Zusammenfassung

30 Jahre Forschung des Tissue Engineering im Bereich der Haut haben zu Zwischenerfolgen geführt, die anfängliche Euphorie in der Hoffnung auf einen Vollhautersatz aus dem Labor ist jedoch gewichen. Insbesondere die Komplexität des dreidimensionalen Hautkonstrukts mit differenzierten, sich ständig erneuernden und untereinander kommunizierenden Zelltypen und Hautanhangsgebilden, bleibt eine Herausforderung an interdisziplinäre Arbeitsgruppen aus Materialtechnikern, Zell- und Molekularbiologen, Biochemikern und klinisch tätigen Verbrennungschirurgen. Es gilt nach wie vor, den derzeitigen Goldstandard der Spalthauttransplantation in der Versorgung großflächiger thermischer Hautläsionen mit seiner minderwertigen Qualität und Neigung zu Narbenbildung und Wundkontraktion durch bessere Hautersatzverfahren abzulösen. Dreidimensionale Matrices mit inokulierten differenzierten Zelltypen im Rahmen des »Skingineering« sind eine erfolgversprechende Aussicht, die teilweise mehr als 50 Jahre alten Techniken der Spalthauttransplantation zu ersetzen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
