Titan oder Stahl als Osteosynthesematerial

Systematische Literaturrecherche über die klinische Evidenz

Zusammenfassung

Hintergrund

Die Auswahl des geeigneten Implantatmaterials – Stahl oder Titan – ist derzeit weitestgehend Ermessenssache des behandelnden Chirurgen und/oder der Institution. Zudem gibt es erstaunliche und nicht erklärbare Unterschiede zwischen den internationalen Märkten.

Ziel der Arbeit

Um zu untersuchen, ob es eine klinische Evidenz für den bevorzugten Einsatz des einen oder anderen Materials gibt, wurden eine systematische Literaturrecherche und Auswertung der Literatur durchgeführt.

Material und Methoden

Die systematische Literaturrecherche erfolgte über die Internetsuchmaschinen PubMed, Cochrane und Web of Science. Vergleichende Studien an erwachsenen Patienten mit Osteosynthese der Extremitätenknochen nach Trauma mit einem Implantat aus Stahl, Titan oder einer Titanlegierung wurden eingeschlossen, wenn sie auch Aussagen enthielten zu Infektionsrate, Inzidenz klinisch relevanter Allergien, Problemen bei der Implantatentfernung oder anderen Komplikationen.

Ergebnisse

Achtzehn Suchergebnisse erfüllten die Kriterien und wurden durch 2 referenzierte Artikel ergänzt. Zusammenfassend gibt es keine ausreichende klinische Evidenz, dass die Anwendung von Titan- oder Stahlimplantaten einen positiven oder negativen Einfluss auf die Frakturheilung hat, zu unterschiedlichen Infektions- und Allergieraten führt oder unterschiedliche mechanische Komplikationsraten aufweist. Probleme bei der Metallentfernung von Titanimplantaten werden mündlich berichtet und beklagt, relevante Studien zu diesem Thema konnten nicht gefunden werden.

Diskussion

Die vorliegende systematische Literatursuche erbrachte keine klinische Evidenz für einen materialbedingten Vor- oder Nachteil von Titan- oder Stahlimplantaten zur Frakturbehandlung. Beide Materialien sind nach der zurzeit vorliegenden Evidenz als gleichwertig zu betrachten. Etwaige Probleme bei der Materialentfernung sind in der aktuellen Literatur nicht abgebildet.
