To the Editor: Biliary atresia (BA) is a progressive disease, and the timing of Kasai portoenterostomy (KPE) is associated with the patient’s prognosis [
1]. The degree of hepatic fibrosis at the time of KPE is predictive of poor outcome after the operation [
2]. The triangular cord sign (TCS), hyperechoic band representing the fibrotic mass at the porta hepatis on ultrasonography, is useful for diagnosing BA [
