It has been said: “Somebody’s leak is a curiosity—one’s leak is a calamity.” And so, enterocutaneous fistulas developing after an operation represent a great therapeutic challenge; but when complicating one’s own operation they seem, and are, an absolute disaster! In this issue of the World Journal of Surgery we present three studies that deal with this frustrating—and however hard we try to prevent it—inevitable complication of abdominal surgery. The principles in the management of enterocutaneous fistulas (ECFs) were established almost 50 years ago and subsequently were “consolidated” in numerous publications and book chapters. So, is there anything “new” or “exciting” in these papers originating from Argentina, Mexico, and the Netherlands? …
