Operative Strategie bei Hiatushernien

Zusammenfassung

Die Indikation zur chirurgischen Behandlung von Hiatushernien unterscheidet zwischen Typ-I- und Typ-II/III/IV-Hernien. Während bei der Typ-I-Hernie für die Indikationsstellung eine nachgewiesene Refluxerkrankung vorliegen sollte, ist die Indikation zur operativen Behandlung von Typ-II/III/IV-Hernien obligat aufgrund der Symptomatik mit Problemen bei der Nahrungspassage sowie aufgrund der teils sehr schwerwiegenden möglichen Komplikationen. Die primären Ziele der Operation sind die Reposition des hernierten Inhalts und eine Hiatoplastik, die eine operative Verengung des ösophagealen Hiatus durch Nahtimplantation beinhaltet. Ergänzend dazu können je nach klinischer Situation weitere Verfahren wie Bruchsackentfernung, Netzimplantation, Gastropexie und Fundoplikation in Erwägung gezogen werden. Es existieren unterschiedliche Ansätze zur Reparatur, die alle ihre individuellen Vor- und Nachteile haben. Eine Anpassung an die spezifische Bedürfnislage des Patienten und die Expertise des Chirurgen ist daher essenziell.
