A 9-year-old boy presented to the emergency department with a 2-day history of fever and left-sided chest pain. His past medical history was unremarkable. Upon physical examination the child was well-appearing, with reduced breath sounds and crackles at the left lung base. A chest ultrasound confirmed the diagnosis of community-acquired pneumonia without signs of empyema; findings of the abdominal ultrasound were unremarkable. Laboratory tests showed elevated C-reactive protein (7.12 mg/dl, normal range <0.5), leukocytosis (17,730/mmc, predominantly neutrophils 15,200/mmc), with normal electrolyte and serum creatinine levels.
