Systemic sclerosis is a rare disease which remains, by many aspects, an enigma and a therapeutic challenge for physicians. It is one of the last “autoimmune” diseases to have a poor prognosis and respond poorly to treatments. The treatment of scleroderma has been very problematic for a number of reasons. Firstly, the genetic basis, although intensely studied, remains unclear and there are no definitive genome-wide association studies published. Second, the pathogenesis of systemic sclerosis is complex, associating several mechanisms including endothelial dysfunction, fibrosis, autoimmunity, and allo-reactivity. Further, all these mechanisms may be inter-related and may change during the disease course. Hence, therapy is a challenge. Further, should therapy focus entirely on specific organ involvement or will there be specific drugs that will modulate the course of disease? Will there ever be drugs that target the complexity of mechanisms present at each disease stage? …

Akuter Schwindel stellt oft eine diagnostische Herausforderung dar. Wie nützlich dabei eine MRT ist, hat eine Studie aus Finnland untersucht. Immerhin einer von sechs Patienten wurde mit akutem ischämischem Schlaganfall diagnostiziert.

Bei schweren Reaktionen auf Insektenstiche empfiehlt sich eine spezifische Immuntherapie

Insektenstiche sind bei Erwachsenen die häufigsten Auslöser einer Anaphylaxie. Einen wirksamen Schutz vor schweren anaphylaktischen Reaktionen bietet die allergenspezifische Immuntherapie. Jedoch kommt sie noch viel zu selten zum Einsatz.
