Zusammenfassung

Die Sonographie ist die wichtigste diagnostische Methode, die bei Verdacht auf eine Lebererkrankung eingesetzt wird, um ein pathologisches Geschehen entweder nachzuweisen (z. B. Metastasierung) oder auszuschließen. Sie ist die beste Technik zur Beurteilung fokaler Leberläsionen (FLL), und dank dynamischer Echtzeitverfahren, hoher Auflösung und maximaler Sicherheit übertrifft sie hier alle anderen Bildgebungsmodalitäten. Darüber hinaus ist sie von unschätzbarem Wert bei der Differenzialdiagnose des Ikterus, bei der Abklärung von Komplikationen der Leberzirrhose sowie bei ultraschallgeführten Interventionen. Für die klinische Hepatologie ist die Sonographie unentbehrlich geworden. Auch für die Beurteilung einer Milzvergrößerung (Splenomegalie) ist die Sonographie die Methode der Wahl. Fokale Milzläsionen sind selten und werden nur bei weniger als 1 % der Untersuchungen entdeckt. Sie lassen sich in neoplastische und nichtneoplastische Erkrankungen unterteilen. Zu den Letzteren gehören ischämische, traumatische und infektiöse Läsionen wie auch Hämatome. Der vorliegende Übersichtsartikel beschreibt das aktuelle Indikationsspektrum der transabdominellen Sonographie für die Diagnostik von Leber- und Milzerkrankungen.
