A 54-year-old female with a history of primary lymphoma of the central nervous system and cancer-related pulmonary embolus developed non-oliguric acute kidney injury (AKI) during admission for cycle 12 of high-dose methotrexate (MTX) and rituximab therapy. Her chemotherapy course had been uneventful until the development of AKI during her two most recent cycles; serum creatinine peaked at 1.6 and 1.7 mg/dl after administration of methotrexate at 4.2 and 4.88 g/m2, respectively. AKI was treated with hydration and supportive management in both cases with improvement in creatinine levels after treatment for each episode. Home medications included levetiracetam and apixaban. …
