Anteriore Glenoidranddefekte der Schulter

Zusammenfassung

Die sog. knöcherne Instabilität des Schultergelenks basierend auf glenoidalen Defekten hat in den vergangenen Jahren zunehmend an Aufmerksamkeit gewonnen. Glenoidrandläsionen können in akute Defekte vom Fragmenttyp (Typ I), chronische Defekte vom Fragmenttyp (Typ II) und glenoidale Substanzverluste ohne knöchernes Fragment (Typ III) unterteilt werden. Die Diagnostik und Einteilung erfolgt im Wesentlichen durch die Röntgeninstabilitätsserie und/oder Computertomographie (CT). Die Therapie wird vom klinischen Erscheinungsbild, Typ der Läsion, relevanten Begleitpathologien sowie dem Alter und Funktionsanspruch des Patienten beeinflusst. Bei manifester Instabilität besteht die Indikation zur operativen Versorgung. Läsionen vom Fragmenttyp ohne signifikante glenoidale Erosion können in der Mehrzahl der Fälle erfolgreich mit arthroskopischen oder offenen Rekonstruktionstechniken behandelt werden. Substanzdefekte ohne knöchernes Fragment können ebenso mit den klassischen Kapsel-Labrum-Refixationstechniken versorgt werden, sofern keine signifikante Defektsituation vorliegt.
Signifikante Knochenverluste erfordern einen knöchernen Aufbau entweder durch eine autologe Beckenkammspanplastik oder einen Korakoidtransfer. Die offenen Techniken zeigen in der Mehrzahl der Fälle zufrieden stellende Ergebnisse. Der Trend geht jedoch auch hier in Richtung arthroskopische Versorgung.
