In the USA, models in surgical training are rapidly evolving due to increasing complexities in healthcare delivery and resident work hour restrictions.
1 The advent of integrated residency training has fundamentally altered the fabric of General Surgery training as integrated residency alternatives exist for Thoracic Surgery, Plastic Surgery, and Vascular Surgery.
2 Almost universally, integrated residencies are more desirable by prospective trainees as evidenced by more competitive match rates. At the same time, changes in physician reimbursement driven by changes in clinical care paradigms and training positions modulate the attractiveness of certain specialties for prospective medical trainees. For example, Radiation Oncology used to be the most competitive specialty for US medical students but now struggles to fill spots in the Match due to eroding salaries and job opportunities for radiation oncologists.
3 Thus, medical specialties should perform periodic self-assessments on the attractiveness of their residency or fellowship training as evidenced by application and match rates. By doing so, key stakeholders can act to preserve the vitality of their specialty by attracting premier talent into the field. …
